This is an exciting time to be connected in the mental health services at Boulder Community Health (Foothills Hospital) as they plan to expand mental health services offered for our community. You will be working within an incredibly skilled, compassionate, and dynamic team of clinicians that are passionate about serving those who suffer with a mental illness/substance abuse disorder. Primary Responsibilities:

  • The inpatient Behavioral Health Care Coordinator/Social Worker (SW) is responsible and accountable for coordination of patient services through an interdisciplinary process, which provides clinical and psychosocial approach through continuum of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization
  • Provides direct social work/case management services to patients and/or families and hospital staff on the inpatient behavioral health unit to ensure needed assessment intervention, support, and referral. Coordinates services with other members of the health care team community wide. Respond to information and referral needs of the community
  • Crisis intervention, comprehensive understanding of mental health diagnosis, brief counseling, working collaboratively with community vendors, identifying resource needs, arrange and facilitate meetings with the health care team as needed, problem solving and encouraging patients toward wellness
  • Integrating the assessment of the need for post-hospital services and determination of an appropriate discharge plan for all Skilled Nursing Facility, Home Care, LTAC cases, alcohol/substance abuse, complex readmission cases, and homelessness cases
  • Facilitates completion of orders and timely discharge of patients, Continuum of Care planning will emphasize education and collaboration with physicians, family members, nursing staff, therapist, and case managers from contracted payors when appropriate to determine discharge plan that will be of maximum benefit to the patient. Involve staff from next level of care in the treatment plan as early as possible to promote continuity and collaboration

Required Qualifications:

  • MSW in Social Work from a CSWE
  • 1+ years experience in an acute hospital setting
  • Clinical knowledge of complex behavioral health patients
  • Knowledge/understanding of Boulder, CO community resources

Preferred Qualifications:

  • LSW
  • LCSW
  • BLS certification
  • Fully licensed clinical social worker

Colorado Residents Only: The salary range for Colorado residents is $56,300 to $110,400. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
