ABOUT THE JOB
The Brooklyn REAL PROS program offers recovery-oriented and rehabilitation services for individuals 18 years and older who are living with mental illness. Participants develop personalized recovery plans to realize life role goals and integrate into the community. The program utilizes evidence-based practices, person-centered individual and group services, and optional vocational and clinical services to assist participants in achieving their goals.
The Social Worker provides recovery-oriented and culturally competent care and services in an OMH-licensed outpatient PROS Program. The responsibilities include:
- Providing person-centered services to a caseload of individual participants, including counseling, intakes, assessments, treatment planning, case management, and discharge planning
- Facilitating a variety of recovery-oriented groups, including groups with established curricula, as well as the creation, development and initiation of new groups
- Coordinating PROS services for participants on caseload in conjunction with all PROS team members, including vocational staff, prescribers, and RN
- Coordinating care with families, friends, medical providers, residential programs, care management, criminal justice, and other community agencies as needed
- Planning, coordinating, and facilitating activities and special events as needed
- Supervision of student interns as needed
- Participating in clinical meetings, case conferences, staff meetings, trainings, and supervision
- Maintaining all documentation according to the standards and time frames established by the Jewish Board, regulatory agencies and/or funding sources
- Maintaining professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures
- Other tasks as needed, eg, group or individual case coverage; kitchen duties, addressing transportation needs, specialization in services for a sub-population (eg, co-occurring disorders, young adults, entitlements and benefits, tobacco use, etc.)
TO QUALIFY
The Jewish Board is currently looking for talented Social Workers of all cultures, religions, races, and gender expressions with the following qualifications:
- Master’s degree in Social Work, (MSW), with a minimum of one year experience treating individuals with mental health disorders required
- LMHC, LMSW, LCSW, or temporary license, with license obtained within one year of hire.
- Experience using an electronic health record and proficiency with Microsoft Office required.
