Central Intake Coordinator Responsibilities
Remote position – must reside in the state of Ohio
The Central Intake Coordinator will be responsible for the clinical and financial assessment of each referral to determine appropriate placement options. Intake Coordinators will work directly with the Hospital Case Manager and Director of Admission to ensure a smooth transition of care for patients discharging from a care setting to our centers.
Essential Functions:
· Establish the patients’ eligibility for care through clinical and financial assessment using the eligibility tools and clinical skills inventory.
· Communicate with clinical teams to remove barrier for admission by utilizing the clinical skills inventory.
· Goal is to have a conversion ratio of 60% or better.
· Communicates to Central Intake Director and Regional Business Development Director appropriately when referrals are a challenge or when assistance is needed to convert referral to admission.
· Verify planned admissions versus actual on assigned daily morning call.
· Communicate with appropriate hospital staff as needed to promote a complete patient assessment and relay information concerning care.
· Maintain a working knowledge of Medicare, Medicaid, Managed Care, and private pay criteria for admission.
· Works with CPAN on pre-certification request
· Develops and maintains working knowledge of all area hospitals discharge and referral procedures.
· Uses appropriate communications and is sensitive to emotional situations.
· Promotes positive customer service relations with referral sources and facility personnel.
· Maintains working knowledge of Point Click Care
· Ensures data on each patient is entered accurately to PCC for MDS billing
· Maintains a working knowledge of online platforms necessary for financial review and state required verifications (HENS, CGS, MITS, OSOR, NSOR)
· Communicates effectively with facility and regional team.
· Coordinates with facility staff to ensure each patient has a smooth transition through the discharge process.
· Sends daily end of day report by 5pm to include planned admissions for each assigned facility.
· Performs other duties as assigned.
This is a remote position responsible for 4 Ayden Healthcare of Toledo locations.
